Job summary

AWD online seeks an Early Years SEND Practitioner to support children aged 0–5 and their families, improving access to right services via Family Hubs and community networks. The role promotes early intervention, inclusion and positive long-term outcomes.

You will assess needs, plan targeted support, and work with health services and SEND teams to deliver drop-ins and family sessions across Family Hubs and partner sites.

Qualifications

• Experience of working with children aged 0–5 with complex needs.
• Experience of working with parents on joint assessment and teaching strategies.
• Good oral and written communication skills across audiences.
• Ability to select and use assessment techniques.
• Plan and review progress for individual children.
• Competent IT skills.
• Knowledge of early child development.
• Understanding of the Early Years Foundation Stage Framework.
• Knowledge of the SEND Code of Practice.
• Excellent organisational and interpersonal skills.
• Ability to manage own time and plan diary.
• Teamwork and ability to work independently.
• Willingness to travel within the county.
• Fluent English.
• Right to work in the UK.

Responsibilities

• Observe children and identify emerging needs using evidence-based tools.
• Deliver universal and targeted early support.
• Support families by agreeing strategies to help them support their child’s needs.
• Help families understand and access inclusion pathways, financial support and community resources.
• Collaborate with health services, early years providers, SEND teams and community partners.
• Provide drop-ins, short-term early support workshops and family sessions.
• Keep accurate records and provide reports in line with service protocols.
• Contribute to multi-agency meetings and reviews.
• Promote inclusion across the Family Hub and wider network.
