Cambridgeshire and Peterborough NHS Foundation Trust is seeking a Band 6 Care Coordinator for Early Intervention in Psychosis in the Cambridge area. The post is suitable for mental health nurses, social workers or occupational therapists with strong psychosis knowledge and a passion for the EI ethos.

You will assess, plan and deliver bespoke care to a designated caseload, working with service users, families and networks.

Formación

• Registered Nurse, Social Worker or Occupational Therapist with registration and appropriate job role.
• Mentorship/ENB or equivalent planned/completed.
• Evidence of ongoing professional development.

Responsabilidades

• Undertake assessments of individuals and families in designated care group with complex health presentations and risk.
• Develop care plans with service users based on recovery principles.
• Offer specialist assessment and advice where required.
• Assess and manage ongoing risks in collaboration with service users and carers.
• Ensure clinical practice aligns with NICE guidelines and Trust policies.
• Provide supervision to junior staff and trainees where appropriate.
