Job summary

Coventry and Warwickshire Partnership NHS Trust is recruiting a Band 6 Care Coordinator to join the Early Intervention in Psychosis multi-disciplinary team in the community. The role focuses on initial assessments, formulation and goal-directed care planning for ARMS/FEP, within a Recovery-focused philosophy.

The post requires post-registration experience with adults/young people in community mental health settings, with ability to travel and work flexibly, and driving Licence is essential.

Qualifications

• Substantial experience in a Community Mental health care setting.
• Current professional registration with HCPC/NMC is required.

Responsibilities

• Communicate with internal and external stakeholders, including service users, carers and partner organisations.
• Provide advised and use initiative, following organisational procedures in routine and crisis situations.
• Apply communication skills to plan, deliver and evaluate care, in records and reports.
• Identify and manage risk and positive risk as part of the caseload.
• Supervise support staff and students.
