Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a Care Coordinator to work with CAMEO service users, developing care and recovery plans to support independence. The role requires understanding of NICE CG178 guidance and EI ethos, and offers opportunities to collaborate with a multidisciplinary team in Peterborough and surrounding areas.

You will assess, plan and implement care, provide specialist input, supervise students, and contribute to safeguarding and wellbeing of service

Qualifications

• Experience in care coordination and care planning in mental health
• Knowledge of NICE CG178 and EIPN Quality Standards CCQI 373
• Ability to work in a multidisciplinary team

Responsibilities

• Develop care plans and recovery plans with CAMEO service users
• Provide specialist interventions and clinical supervision to staff and students
• Promote safety and wellbeing of service users and families
• Collaborate with partner agencies and involve carers and families
• Adhere to the early intervention ethos and work with under 17s as required
