Join the NHS as a care coordinator in the Forensic Intellectual and Neurodevelopmental Disabilities team. This role involves providing care coordination for individuals with learning disabilities and Autism, managing high-risk clients, and ensuring compliance with legal and statutory obligations. You will work collaboratively with a multidisciplinary team to develop and implement care packages, emphasizing service user involvement and support.
This is an exciting and unique opportunity to join the newly developed Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in Oxleas.
You will work within the Forensic Directorate, providing care coordination and support to individuals with learning disabilities and Autism, in accordance with the team's operational policies and inclusion criteria.
Your role includes providing intensive supported placements for men and women with extended inpatient stays to develop skills for community living, focusing on vocational, occupational, and social inclusion through support, community access, work experience, training, and employment support. Emphasis is placed on service user involvement and peer worker support.

Manage a caseload of high-risk clients, balancing therapeutic relationships with legal and statutory obligations. Conduct assessments, develop and evaluate treatment plans, and ensure reviews adhere to Trust CPA policies. Provide care coordination for clients under statutory supervision, including those discharged conditionally by the Home Office. Develop evidence-based interventions, act as social supervisor and care coordinator, and ensure clinical effectiveness through flexible engagement strategies. Prepare social circumstances reports, including for the Mental Health Review Tribunal. Be prepared to encounter distressing and challenging accounts of offending behavior, including extreme violence or sexual assault.
