An established industry player is seeking passionate Transition Support Workers to join their team in Nottingham. We are looking for two Transition Support workers who will be based at our school in Nottingham City Centre, Sutherland House School. This is an exciting new project launching in April 2025 to support autistic young people and adults across the East Midlands.
We currently have two vacant posts, one role focusing on children and young people and one role focusing on adults. Both roles will focus on supporting young people and adults transition into adulthood to secure meaningful long-term outcomes.
The purpose of the role is to provide a consistent presence in the life of young people aged 14-25 years of age to enable a supportive transition into adulthood.
The role will enable long-term positive outcomes, provide signposting for support available including housing and benefits and assisting in employment or further education opportunities for autistic young people and adults including supporting access to work.
What will you do
Our Transition Support Workers will provide one-to-one support to autistic individuals. This will include both face-to-face and online meetings and the securing of long-term outcomes for those who access the Transition Support Package.

The post-holder should be someone that is passionate about helping autistic individuals navigate the barriers that young peoples may face when transitioning from children s services to adult services.
