The Community Therapy Team works with people with a wide range of disabilities and acute or long-term conditions, striving to deliver excellent patient-centred care. Our work is varied and stimulating, covering admission prevention, supporting discharge, and rehabilitation in community settings through medical, social, and public health approaches. The team works within the broader Health and Social Care team alongside Community Nursing and Adult Social Care colleagues.
We are a positive and proactive team that adapts to changing patient needs and service demands. If you are flexible, creative, enjoy working in a multidisciplinary team, and are interested in developing yourself and others, this could be the opportunity for you.
Working Pattern: up to hours per week, including participation in a 7-day service rota.
