The Home First Single Point of Access Team is looking to recruit 3 permanent social workers to work a shift pattern over 7 days a week. Our work involves individuals aged 18 and above, offering a varied and dynamic caseload with diverse needs.
The ideal candidates will hold a social work degree and be registered with Social Work England. We seek strong team players with a proactive approach, suitable for both experienced workers and Newly Qualified Social Workers (with an ASYE year offered).
The successful candidates will be highly motivated, capable of undertaking Care Act Assessments, support/care planning, and commissioning services with a focus on person-centered outcomes. They should be able to provide social work supervision and have experience in social work management. Independence in travel across North Lincolnshire is required. Knowledge of adult social care legislation and frameworks is essential.
