Job summary

Northern Care Alliance NHS Foundation Trust in Salford invites applications for a full-time permanent Band 3 Screening Officer in the District Nursing Contact Centre. You will support the District Nursing Team within the Adult Community Services Division.

You will process referrals, discharge patients, manage inboxes, and relay messages to clinical coordinators. The service runs 08:00–18:00, 5 shifts per week, with Monday to Sunday coverage.

Qualifications

• Customer service and communications with patients and staff.
• Accurate data entry into multiple systems.
• Ability to follow escalation policies and handle sensitive data.

Responsibilities

• Screen and process referrals by booking clinic appointments or home visits.
• Input patient information accurately into databases and manage inboxes.
• Relay messages to clinical coordinators in a timely manner.
• Prioritise workload and communicate effectively with team members.
• Escalate urgent referrals following policy and maintain patient confidentiality.
