Job summary

Oxford Health NHS Foundation Trust is recruiting for a District Nurse Triage role based in the community setting. The post involves assessing referrals via phone and email, prioritising care levels, and coordinating urgent, same-day and routine visits for patients in the community.

You will work closely with District Nurse teams, provide clinical advice where needed, and contribute to ongoing development of the triage service and community care delivery.

Qualifications

• IT literacy and basic keyboard skills are required.
• Demonstrates understanding of clinical governance requirements.

Responsibilities

• Triage referrals received by telephone and email.
• Identify appropriate level of care and urgency using clinical judgement and risk assessment.
• Arrange and coordinate urgent, same-day and routine visits for patients in the community.
• Provide clinical advice and support where face-to-face visits are not necessary.
• Collaborate with District Nurse teams and wider community services for safe care coordination.
• Contribute to development of the District Nurse Triage Desk and future delivery of community care.
