Job summary

First Community Health & Care is recruiting a Band 6 District Nurse with Specialist Practice Qualification to join our South Tandridge team providing NHS community care across a 7 day service.

The role is part-time (22.5 hours, potentially 30 hours) across a 365 day service, with travel across the patch and weekend/Bank holiday shifts. You will work with MDT colleagues to deliver high-quality nursing care in patients' homes.

Qualifications

• RN1 registered with NMC and current practice
• Specialist Practice Qualification in District Nursing required
• Mentor- Practice assessor and supervisor qualifications preferred
• Experience in community nursing and MDT working recommended

Responsibilities

• Manage and deliver high-standard nursing care in the community
• Continue caseload management and work with neighbourhood teams
• Assess, plan, implement and evaluate care episodes
• Line manage staff within the team and contribute to development
• Work 365 days a year with shifts between 8am and 6pm and travel across the patch
