Job summary

Lincolnshire Community Health Services NHS Trust is recruiting a District Nurse to join the Horncastle and Spilsby area on a developing Band 6/Band 5 post.

The role requires Community Specialist Practitioner qualification and experience managing a caseload while leading a team to deliver high-quality patient care. There is a fixed-term development option for one year and a 7-day service with shifts 8:00–18:30.

Qualifications

• Current NMC registration
• Specialist Practitioner qualification in District nursing
• Evidence of Continuous Professional Development
• Management or Leadership Qualification

Responsibilities

• Hold a caseload of patients with varying health needs, coordinating care delivery.
• Lead the community nursing team through change and transformation of services.
• Travel to patients' homes and clinic settings as part of a rural Lincolnshire post.
• Be available to work across a 7 day service with shifts from 8:00 to 18:30.
