Job summary

Buckinghamshire Healthcare NHS Trust is recruiting for a Community case manager/district nurse in Amersham. The role is part-time (30 hours/week) with driving essential for home visits.

You’ll provide nursing interventions, develop skills in medicines management and wound care, and work within a multidisciplinary team to deliver high-quality care. The post requires a Band 6 level nurse with NHS experience in community settings.

Qualifications

• Registered Allied Healthcare Professional with current registration.
• Nurse degree/diploma in health/nursing studies or related subject.
• Evidence of recent Continuous Professional Development.

Responsibilities

• Provide nursing interventions to patients in their home environment.
• Manage caseload with long-term conditions and complex care needs.
• Undertake referral management and develop case management programmes.
• Deliver medicines management, wound care, palliative and end-of-life care as appropriate.
• Work within a multi-disciplinary team to ensure high-quality service delivery.
