Job summary

Northern Care Alliance NHS Foundation Trust is seeking a District Nurse/Charge Nurse for its Community Nursing Team in Rochdale North. You will deliver holistic, patient-centred care in patients' homes, planning and coordinating services to support independence and quality of life.

You will work autonomously within a supportive multidisciplinary team, providing leadership, clinical decision-making, and teaching to colleagues and carers.

Qualifications

• Registered Nurse with current NMC registration (Adult/General)
• Experience in community or district nursing preferred
• Evidence of reflective practice and professional portfolio

Responsibilities

• Assess, plan, and manage care for a diverse caseload in patients' homes
• Coordinate care with multidisciplinary teams and families
• Deliver holistic, person-centred care and promote independence
• Support patients with end-of-life care and rehabilitation goals
