You will deliver high-quality, patient-centred care to people in their own homes, assess and coordinate care, and work with a multidisciplinary team to enable independence.

This role supports a seven-days-a-week service across Greater Manchester, with opportunities for leadership development and impact on patient outcomes.

• Coordinate care with the Community Nurse Team Leader and multidisciplinary teams.
