Job summary

East Sussex Healthcare NHS Trust is seeking a motivated District Nurse to join our community team. You will assess patients in their own homes, create and evaluate care plans, and manage a caseload with support from the Clinical Lead.

Leadership of a team of Registered Nurses and HCAs is required, with flexibility across teams to meet service demands. The role operates within core service hours 08:00–18:00, seven days a week.

Qualifications

• DN SPQ is an essential criteria.
• RN Part 1 on NMC register is required.
• Nurse prescriber / V100 status desirable.
• Teaching & assessing module or commitment to undertake is required.

Responsibilities

• Assess patients in their homes and develop/ evaluate care plans.
• Manage a caseload with support from Clinical Lead.
• Provide leadership and line management to RN and HCAs.
• Work flexibly across teams to meet service demands.
