Job summary

East Sussex Healthcare NHS Trust is seeking a District Nurse to join our community team. You will assess patients in their homes, developing, implementing and evaluating individualised care plans, and manage a caseload with support from the Clinical Lead.

The role requires a District Nursing qualification (DN SPQ), RN Part 1, and Nurse prescriber status; you will provide leadership to a team of RNs and HCAs, working flexibly to meet service needs across the Cross site East/West base.

Qualifications

• DN SPQ district nursing qualification is essential.
• NMC registration (RN Part 1) required.
• Nurse prescriber / V100 status desirable.

Responsibilities

• Assess patients in their homes and develop, implement and evaluate care plans.
• Manage a caseload with support from the Clinical Lead.
• Provide leadership to a team of RNs and Healthcare Assistants.
• Work flexibly across teams to ensure continuity of care.
