Qualifications

• Registered Nurse with post-registration qualification and relevant specialist training in district nursing.
• Experience and leadership in community nursing settings.
• Independent prescribing qualification or progress towards.
• Master’s level qualification (Nurse Practitioner) or equivalent advantageous.

Responsibilities

• Monitor quality of care and identify service development opportunities.
• Provide clinical leadership across the community nursing caseload.
• Lead on integration and partnership with health and social care colleagues.
• Support and develop the nursing team to deliver evidence-based care.
