Aberdeenshire Council seeks an Enablement and Support Co-ordinator to support the Discharge to Assess (D2A) pathway within Aberdeenshire Health & Social Care Partnership. The role supports patients leaving hospital to home or community settings and will work within a multidisciplinary team to ensure timely, appropriate discharge while safeguarding vulnerable groups.

Based at ARI within the discharge hub, the post involves travel across the Garioch and Formartine area and offers hybrid working

Qualifications

• PVG Scheme/Disclosure checks required for regulated work with children and/or protected adults.
• Two references (one from current or most recent employer) required.

Responsabilités

• Contribute to the Discharge to Assess (D2A) care delivery model.
• Collaborate with multi-disciplinary teams to support patient discharge planning.
• Travel across the Garioch and Formartine area; hybrid role with some home working.
