Job summary

Midlands Partnership NHS Foundation Trust is seeking an Advanced Clinical Practitioner to support Discharge to Assess (D2A) services at Haywood Community Hospital. You will lead advanced clinical assessment and care coordination for patients moving through D2A pathways, coordinating with medical teams and partners to promote safe, timely discharges.

This role requires autonomous practice, clinical decision-making and prescribing where appropriate, within the NMC/HCPC framework, and a commitment

Qualifications

• Experience of working within D2A Services.
• Ability to work as an autonomous practitioner.
• Approved MSc in Advanced clinical practice.

Responsibilities

• Provide advanced clinical assessment, diagnosis, treatment planning and care coordination for D2A pathway patients.
• Work with medical teams, therapists, nursing staff, social care and external partners to ensure seamless patient journeys.
• Perform and interpret diagnostic procedures and initiate appropriate investigations and treatments.
