Job summary

Angus Council in Forfar is seeking frontline care staff to join the enablement team, providing short-term support for people being discharged from hospital and those needing in-home assessment.

You'll develop and amend support plans, monitor goals, manage risk, and update case notes while empowering individuals to maintain outcomes. A full driving licence and access to a vehicle are essential; travel across Angus is required.

Qualifications

• Ability to become a member of the PVG Scheme.
• Awareness of Adult Support and Protection and National Health and Social Care Standards.
• Willingness to travel across Angus and work a 4 on / 4 off shift pattern.

Responsibilities

• Develop and amend support plans to deliver assessed needs.
• Proactively support planning and monitor goals and outcomes.
• Manage risk assessments and update case notes.
• Empower individuals to reach goals and support carers.
• Undertake personal care tasks and promote nutrition, hydration, mobility and independence.
• Travel across Angus as part of duties; driving licence and access to a vehicle essential.
• Temporary role for up to 12 months; 35 hours per week, 4 on/4 off shifts.
