Job summary

Peabody Community Foundation is seeking a compassionate Resettlement Support Worker to assist people leaving hospital who may be homeless or face housing barriers. Based in Stoke Newington, the role focuses on settling customers into the community, developing life skills, and moving to longer-term housing.

The post involves varied duties, including liaising with hospitals, housing providers, and agencies to support positive outcomes.

Qualifications

• Experience supporting vulnerable adults in housing, homelessness, resettlement or similar.
• Knowledge of housing management and tenancy sustainment.
• Understanding of safeguarding, health and safety and risk management.
• Experience completing support plans and risk assessments.
• Knowledge of trauma-informed approaches.
• Ability to support practical life skills and independent living.
• Experience working with external agencies and stakeholders.
• Good IT skills and accurate record-keeping.

Responsibilities

• Provide high-quality housing management and resettlement support to customers.
• Help customers access benefits, healthcare, and community services.
• Provide guidance on budgeting and independent living.
• Work with hospitals, housing providers, and community organizations to achieve positive outcomes.
• Identify suitable long-term accommodation and support transitions.
• Carry out health and safety checks, inspections, and welfare visits.
• Respond to safeguarding concerns and manage risk.
• Maintain accurate records and risk assessments.
