Job summary

Hertfordshire County Council in Hemel Hempstead is seeking a Post Hospital Review Team Manager to lead an Integrated Discharge Service, coordinating multi-disciplinary teams to support people on reablement and discharge to assess pathways after hospital discharge.

You will drive performance, ensure safeguarding and person-centred practice, work with NHS colleagues, and manage resources while promoting independence and system flow.

Qualifications

• Experience leading teams in busy health/social care settings.
• Strong safeguarding knowledge and practice.
• Ability to manage priorities and drive performance improvements.

Responsibilities

• Lead day-to-day practice within the team while ensuring compliance with duties, policies and procedures.
• Manage team performance to deliver discharge-to-assess pathways, reviews and assessments.
• Inspire and develop Social Workers and Community Care Officers to deliver person-centred care.
• Build strong relationships with hospital, community services and partners to support integrated working.
• Oversee resources and budgets, monitor quality and drive service improvements.
• Act as lead manager for safeguarding enquiries and high‑profile concerns.
