Job summary

Wakefield Hospice seeks an experienced leader to head the integrated Family Care Team, uniting rehabilitation, reablement, discharge planning, family support and bereavement services. You will drive strategic development of coordinated, person‑centred care across the hospice journey.

The post holder will optimise patient flow, oversee routine discharges with multidisciplinary autonomy, and manage complex transitions in collaboration with internal and external partners.

Qualifications

• Experienced registered health or care professional with credibility to lead across boundaries.
• Strong understanding of complex discharge planning and integrated working.
• Proven ability to drive service improvement and staff development.

Responsibilities

• Lead the Family Care Team to optimise patient flow and discharge practice.
• Develop pathways and coordinate cross‑hospice and external partnerships for complex care transitions.
• Champion evidence‑based continuous improvement and contribute to clinical leadership activities.
