Job summary

Surrey and Borders Partnership NHS Foundation Trust seeks a Discharge Practitioner in the Flow Optimisation function. Based mainly at Farnham Road Hospital or Silverwood, you will coordinate discharge activity across wards and support discharge progression.

You will work closely with Senior Discharge Practitioners, Bed Flow Teams, HTT, CMHRS, Local Authorities and housing/commissioning partners to identify barriers and expedite safe discharges.

Responsibilities

• Attend ward rounds and MDT/discharge meetings.
• Coordinate referrals and community follow-up.
• Support 72-hour follow-ups and home visits where appropriate.
• Maintain accurate SystmOne records and track discharge actions.
• Progress discharge plans from admission to discharge; escalate delays.
