Job summary

Midlands Partnership NHS Foundation Trust in Stafford is seeking a NMC Registered Band 6 Senior Nurse Co-ordinator for the Discharge to Access (D2A) Home First Service. The role covers Stafford and surrounding West Staffordshire, with 24/7 shift patterns and travel across a large area.

You will assess, plan and deliver care, supervise students, and work with a multi-disciplinary team to ensure high-quality community discharge and rehabilitation. The post is permanent with full-time hours.

Qualifications

• RGN qualification with current UK registration.
• Experience in multi-disciplinary healthcare settings.
• Understanding of discharge to assess pathways and timelines.
• Ability to make independent clinical decisions with sound judgment.
• Commitment to continued professional development.

Responsibilities

• Assess, plan, and deliver care; support peers and team leader.
• Triage referrals for discharge-to-assess pathway per criteria.
• Holistic assessment and care planning (Chronic disease, Palliative care).
• Supervise pre-registration students.
• Collaborate with health and social care professionals and agencies.
• Foster high standards of practice and productive teamwork.
