Stockport NHS Foundation Trust is seeking a Ward tracker to provide a comprehensive discharge service, ensuring timely discharges for patients no longer requiring acute care. The post focuses on liaison between the Hospital Discharge Team, the Discharge to Assess Hub and wards to safeguard safe patient turnover.

Responsabilités

• To support the Discharge Team in facilitating timely discharges for complex patients
• Attend Whiteboard rounds and ward rounds, identify actions required and actively manage agreed discharge plans for patients
• Assist in the completion of discharge check lists to ensure accurate discharge planning is evident, under the direction and supervision of the registered nursing staff
• Transfer patients to the discharge lounge if required
