Job summary

Stockport NHS Foundation Trust in Stockport is seeking a Ward Tracker for Discharge Services on a permanent, full-time basis (37.5 hours per week across 7 days). The post focuses on facilitating timely discharge of patients no longer requiring acute care, coordinating with the Discharge Team, the Discharge to Assess Hub and hospital wards.

Qualifications

• NVQ/NVQ-equivalent healthcare qualifications required or in progress.
• GCSEs or equivalent educational background.
• Willingness to study for Care Certificate.
• Experience working in healthcare or care settings is desirable.

Responsibilities

• Support Discharge Team to facilitate timely discharges for complex patients.
• Attend rounds, identify actions, and manage discharge plans.
• Complete discharge checklists under supervision of nursing staff.
• Transfer patients to discharge lounge when required.
