Job summary

Mid and South Essex NHS Foundation Trust is seeking an IDT Nurse Assessor to join the Integrated Discharge Team at Broomfield Hospital in Chelmsford. You will assess patients for safe discharge, plan holistic care, and coordinate referrals to community services to ensure smooth transitions from hospital to home.

The role focuses on timely, patient-centred discharge planning, collaboration with families and multi-agency partners, and optimizing patient flow within the acute hospital.

Qualifications

• NMC Registration with current pin.
• Evidence of proactive and continuous personal and professional development.

Responsibilities

• Assess patients medically optimised for discharge and coordinate timely referrals.
• Develop and coordinate holistic discharge plans with patients, families and multi-agency colleagues.
• Act as liaison between hospital and community services to ensure safe transfers of care.
• Identify ongoing care needs and remove barriers to discharge to maintain patient flow.
• Support patients with complex healthcare needs and high risk of unplanned hospital admission.
