Job description

A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our Peterborough Team, working full time 37.5 hours per week. If you possess compassion, empathy, and the ability to work autonomously, this is the opportunity for you.

You will work as part of the multidisciplinary team with the aim of preventing admissions and providing ongoing care for patients with diabetes within the community.

You will also be responsible for managing a caseload of patients with complex diabetes needs, using evidence-based practice and patient-centered goals to assess, plan, implement, and evaluate interventions.

The role includes promoting a better understanding of diabetes for patients, their families, carers, and other involved agencies through education, support, and counseling.

Key Responsibilities:
  1. Act as a key worker, coordinating multidisciplinary care as appropriate.
  2. Provide expert support, advice, and education to raise awareness of the needs of patients with diabetes.
  3. Undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues.
  4. Work closely with primary and secondary care to ensure a safe and effective patient pathway for all diabetes patients under the community diabetes team.
