An exciting opportunity has arisen for a band 7 Heart Failure Specialist Nurse to join our community-based service.
We are looking for a dynamic, highly trained nurse to join our well-established service covering Leicester, Leicestershire, and Rutland.
Our community-based specialist service aims to improve outcomes for patients living with heart failure by working as part of a wider MDT. We strive to deliver evidence-based pharmacological and holistic care tailored to each patient, reduce hospital admissions and costs, enhance quality of care, quality of life, and patient experience. We support self-management of heart failure and bridge primary and secondary care.
The successful candidate will work around Leicester City, with mobility within Leicester, Leicestershire, and Rutland required. You will see patients in community clinics and their homes; a full UK driving license is essential. We operate a 7-day service, including a virtual ward at weekends. You will participate in service development and support and teach other staff.
