Job overview
We are seeking a dynamic, motivated, and enthusiastic experienced diabetes nurse to join the Diabetes service at Guy’s & St Thomas, including our Lambeth and Southwark Community Sites.
Our department is highly patient-focused, supporting innovation and service development to continually improve care for people with diabetes. We are a busy tertiary specialist diabetes centre with over 2000 people with type 1 diabetes and 600 on Continuous Subcutaneous Insulin Infusion (insulin pumps). We also have a forward-thinking community service across Lambeth and Southwark, actively supporting patients with T1 and Type 2 diabetes. Education of patients and healthcare colleagues is essential in this role.
The ideal candidate will have experience in diabetes specialist nursing across any care pathway. If you enjoy multidisciplinary teamwork in a busy, progressive environment committed to improving care, this position is suitable. We seek a nurse who is flexible, dynamic, self-motivated, reliable, and passionate about diabetes and nursing.
The successful candidate will join a large diabetes nursing team providing specialist care across integrated pathways in in-patients, out-patients, and community/intermediate care settings.

The primary role involves acting as a highly specialised diabetes nurse, working autonomously to assess, treat, prescribe for patients with diabetes, and advise non-specialist practitioners on management plans. The team operates across community and secondary care services.
This is a hybrid hospital/community DSN post within GSTT Diabetes service.
The post holder will develop, implement, and evaluate a seamless specialist service, ensuring high standards of clinical care. They will act as a lead specialist, providing advice, education, and support to healthcare professionals and patients, and will undertake clinical audits and research.
The role includes contributing to clinical practice development, research, and standards of care, including policy development, and service improvements in collaboration with colleagues. The post also emphasizes patient safety, dignity, and human rights through person-centered care principles.
