Job summary
Are you a band 5 or current band 6 nurse with an interest in diabetes, looking to expand your knowledge and skills in diabetes care and management?
If so, the Leeds Community Diabetes Service in Leeds has a role for you within our team.
We have an exciting opportunity for a diabetes specialist nurse to join our multidisciplinary team, comprising nurses, dietitians, GPs, diabetes facilitators, and pharmacists. If successful, you will work across Leeds, providing face-to-face and telephone clinics, home visits, and education groups for patients with type 2 diabetes.
This role is suited for someone enthusiastic and committed to delivering excellent diabetes care, eager to develop their expertise further. Our team is dedicated to staff wellbeing and professional growth. The successful candidate will be supported to complete Non-Medical Prescriber (NMP) training if not already obtained, as this is a core requirement of the role.
The role requires excellent communication skills to undertake complex assessments of patients with diabetes, implement treatment plans, and review progress, all while promoting self-management for individuals with diabetes.
Having a car or suitable alternative transport is essential to perform the duties effectively, as public transport may not suffice.
