We are seeking an experienced andhighly motivated Diabetes Nurse to take a leading role in managing diabetescare within our practice. The successful candidate will work collaborativelywith our existing Diabetes Specialist Nurse and Diabetes Lead GP to ensure ourpatients receive high-quality, evidence-based care. The role will involvedirect patient management, education, and service development, as well assupporting and mentoring other members of the clinical team.

Main duties of the job
  • Lead and coordinate the diabetes service within the practice, ensuring high standards of care.
  • Work alongside the Diabetes Specialist Nurse and Diabetes Lead GP to manage complex diabetic patients.
  • Provide expert clinical advice, assessment, and treatment planning for patients with diabetes.
  • Deliver structured diabetes education for patients, promoting self-management and lifestyle interventions.
  • Support and mentor other healthcare professionals in diabetes care, including practice nurses, GPs and Pharmacists.
  • Develop and implement evidence-based protocols and guidelines in line with national and local guidance.
  • Undertake regular audits and quality improvement initiatives to enhance diabetes care.
  • Contribute to the management of long-term condition clinics and provide leadership in proactive patient care.
  • Engage in collaborative working with local diabetes services, secondary care, and community teams.
  • Stay up to date with best practices and advances in diabetes care through continuous professional development.
