We are seeking an experienced and highly motivated Diabetes Nurse to take a leading role in managing diabetes care within our practice. The successful candidate will work collaboratively with our existing Diabetes Specialist Nurse and Diabetes Lead GP to ensure our patients receive high-quality, evidence-based care. Main duties of the job

- Provide expert clinical advice, assessment, and treatment planning for patients with diabetes.
- Deliver structured diabetes education for patients, promoting self-management and lifestyle interventions.
- Support and mentor other healthcare professionals in diabetes care, including practice nurses, GPs and Pharmacists.
- Develop and implement evidence-based protocols and guidelines in line with national and local guidance.
- Undertake regular audits and quality improvement initiatives to enhance diabetes care.
- Contribute to the management of long-term condition clinics and provide leadership in proactive patient care.
- Engage in collaborative working with local diabetes services, secondary care, and community teams.
- Stay up to date with best practices and advances in diabetes care through continuous professional development.
