District Nurse Team Lead - Surrey Downs H&C Surrey Downs Health and Care

Position Overview

We are seeking a Band 7 District Nurse Team Lead to join the Leatherhead PCN team on a full-time (37.5 hours/week), permanent basis. The successful candidate will work within an established district nurse team, collaborating closely with the multidisciplinary team across the PCN and Surrey Downs Health and Care.

Key Responsibilities
  1. Provide leadership within the District Nursing Team, including assessment, care planning, and evaluation of patient needs.
  2. Facilitate educational programs and support staff development.
  3. Lead research-based practice initiatives aligned with Surrey Downs Health and Care objectives.
  4. Promote person-centered care and respect patient dignity and privacy.
  5. Ensure appropriate staffing, resource management, and adherence to policies and standards.
  6. Participate in clinical governance, risk management, and quality improvement activities.
  7. Support workforce planning, recruitment, and retention efforts.
  8. Maintain accurate documentation and data entry.
  9. Engage in research, development, and continuous improvement initiatives.
