Job description

The Projects and DES Care Co-ordinator will provide high-quality administrative and coordination support to ensure seamless delivery of PCN services, with a focus on Direct Enhanced Services (DES) implementation, patient care continuity, data quality and service improvement.

Main duties of the job

We arelooking for a motivated andwell-organised Projects and DES Care Co-ordinator to join Surrey Heath PrimaryCare Network (PCN). This varied role supports the coordinationand delivery of Direct Enhanced Services (DES) and key PCN projects, ensuringhigh-quality, patient-centred care across our practices.

You will work closely withclinical and administrative teams to help patients navigate health and careservices, supporting continuity of care and smooth communication across thenetwork. The role includes maintaining EMIS clinics and user access, runningreports, managing shared inboxes, coordinating clinics and Enhanced Accesssessions, and ensuring data quality and compliance.

You will also assist withaudits, quality returns, and PCN projects such as Dermatology, SDUC, and HeartHealth Checks, updating SOPs and TeamNet content as processes evolve.

We are Surrey Heath Community Providers Limited, which is a federation of 7 GP practices across 10 sites, covering a population of over 100,000 patients across Surrey Heath. We began in 2016 and now employ around 200 members of staff across our clinical and corporate support teams.

Our services include Surrey Heath Primary Care Network, Primacy workflow management and Upper Gordon Road Surgery. Person Specification
Experience
  • Experience of working with healthcare professionals and/or within the NHS or social care
  • Experience coordinating with multiple stakeholders or individuals to meet defined outcomes
  • Experience providing advice or signposting patients to appropriate services
  • Experience in managing data quality, reporting or audit activity
  • Experience supporting projects, service delivery or quality improvement work
  • Knowledge of safeguarding interventions and awareness of the Mental Capacity Act
  • Experience managing shared inboxes or coordinating administrative workflows
  • Understanding of Direct Enhanced Services (DES), Enhanced Access and EHCH frameworks
  • Knowledge of data protection, confidentiality and information governance standards
  • Awareness of local community groups and social prescribing services that support wellbeing
  • Understanding of relevant Health and Social Care legislation and crisis intervention pathways
  • Understanding of disease prevention, health promotion and NHS Choices
Skills and Abilities
  • Excellent verbal and written communication skills; able to communicate with patients, carers, clinicians and managers
  • Strong IT literacy with Microsoft Office (Word, Excel, Outlook) and ability to learn new systems quickly
  • Confident in using EMIS and TeamNet for data entry, reporting and document management
  • Proven ability to plan, prioritise and manage own workload to meet deadlines
  • Strong organisational skills and attention to detail when handling data and reports
  • Ability to analyse and interpret information and present findings clearly
  • Able to build and maintain strong professional relationships across teams and agencies
  • Understanding of social determinants of health and how these can be addressed with patients

Qualifications
  • 4 GCSE's (Grade C and above) English and Maths or equivalent
  • Relevant NVQ level 3 qualification or equivalent experience
