Job summary

The post holder will work closely with and deputise for the ward manager in their absence. Be an active member of the multi-disciplinary team, supervise, teach, and instruct junior staff, Nursing Assistants, Activity Coordinators, and STR Workers in assessing, planning, implementing, and evaluating patient care. Working independently, the post holder will be responsible for developing and coordinating individual assessment and care plans as agreed with the multi-disciplinary team. The post holder will adhere to Trust policies, data protection, and Health and Safety standards.

Main duties of the job

1. Participate as an effective member of the MDT and be responsible for developing and implementing a patient-centered approach to care: assessing, developing, implementing, and evaluating the care needs of patients and their carers.
2. Interpret the significance of clinical observations, act upon them, and communicate information to other multi-disciplinary team members after making necessary clinical judgments, in line with clinical protocols.
3. Determine nursing priorities and plan patient care accordingly, including full risk assessments, and inform the Ward manager when changes in circumstances may affect standards of care.
4. Meet the physical, psychological, social, and cultural needs of the individual and their family, using research-based knowledge and skills to maintain the individual's identity.
5. Recognize the needs of the family/carers, ensuring their involvement in care where appropriate, whilst offering support and education with empathy.
6. Ensure high standards of nursing care are maintained, participate in setting and reviewing standards, nursing audits, and support staff in delivering high-quality care.
