Job summary

Cinnamon is recruiting a Care Coordinator to aid the General Manager and Deputy Manager in the daily running of Earlsfield Court, a 72-bed home, around the clock. You will maintain high quality care standards and lead the team to ensure a person-centred approach, including supervising and covering duties in the Deputy Manager's absence.

You should have a relevant qualification and at least two years in long-term care, with COSHH, CQC and health & safety knowledge.

Qualifications

  • Two years' experience in long-term care or similar industry.
  • Awareness of COSHH and CQC regulations.
  • Working knowledge of Health and Safety legislation.

Responsibilities

  • Co-ordinate the resident assessment process, ensuring resident involvement and integration into the care plan.
  • Communicate all relevant information to achieve a person-centred approach.
  • Lead hands-on with the team to maintain appropriate levels of care; lead by example.
  • Prepare weekly A&I report and address concerns to improve quality outcomes.
  • Oversee Safe Medication Management programme with robust auditing including pharmacy processes.
  • Assist with supervision meetings and yearly performance reviews, following Cinnamon Care processes.
  • Ensure staff coverage through scheduling and daily assignments in Deputy Manager's absence.
  • Conduct formal procedures with the Deputy Manager such as investigations and disciplinaries.

Earlsfield Court is a stunning 72 bedded luxurious residential and dementia care home providing the very best in care and support.

We are recruiting a Care Coordinator to assist the General Manager and Deputy Manager in the day to day running of the home on a 24- hour basis. You will help to maintain and improve quality Care standards and also stand in for the Deputy Manager across the whole range of their duties in their absence.

The Care Coordinator sets the standard for Team Leaders, Senior Care and Care Assistant responsibilities, including ensuring excellence in resident care, providing outreach and support to families.

Main Responsibilities
  • Co-ordinate the resident assessment process, ensuring involvement of the resident in identifying needs and preferences, and integration of all information into the care plan.
  • Ensure that all relevant information in respect of each resident is communicated as appropriate to achieve delivery of a person-centred approach.
  • To work hands-on with the team to ensure the appropriate levels of care are always in place. Lead by example.
  • To prepare the weekly A&I report and action any concerns noted to improve quality outcomes.
  • Oversee the Safe Medication Management programme, ensuring robust auditing procedures including those surrounding pharmacy processes, and identify any training needs.
  • To assist to complete all supervision meetings and yearly performance reviews with staff in the department, ensuring that the Cinnamon Care process is always followed.
  • Ensure appropriate staff coverage through effective staff scheduling and completion of daily assignments in the absence of a Deputy Manager.
  • To conduct formal procedures alongside the Deputy Manager, such as investigations and disciplinaries.
Person Specification
  • Be working towards or have an NVQ 4 in Care or a Level 5 in Care Management or a Registered Managers Award or QCF Level 4 Diploma in Management.
  • Two years' experience in long-term care or similar industry
  • Plan, implement, evaluate, and clearly document care delivery
  • Team player, decisive, initiative-taking, initiative-taking, flexible, and adaptable
  • Awareness of COSHH and CQC regulations
  • Working knowledge of Health and Safety legislation
