Job summary

South Coast Nursing Homes in Worthing seeks a Dementia Wellbeing Coordinator at Berkeley Lodge. You will develop personalised care and activity plans grounded in residents' life histories and preferences, coordinating reminiscence, sensory sessions and gentle exercise to promote dignity and independence.

You will monitor cognitive and emotional wellbeing, adjust strategies as needed, and maintain accurate records.

Qualifications

• Professional or personal experience supporting individuals living with dementia.
• Experience working with older people, ideally in a care home or community setting.
• Experience in planning and facilitating person-centred activities or cognitive stimulation therapy.

Responsibilities

• Develop, manage and implement individualised care and activity plans based on residents' histories and preferences.
• Coordinate diverse therapeutic activities that promote dignity and independence.
• Monitor residents' cognitive and emotional wellbeing and maintain accurate records.
