Job summary

Alzheimer's Society in Bristol area is seeking a Dementia Navigator to support people affected by dementia. The role is 35 hours per week, Monday to Friday, with a salary of £25,391 to £27,450 per annum on a permanent contract. You will work across Bristol, visiting homes, meeting GPs and connecting with local communities.

You'll balance a busy caseload, listen to individual needs and provide information and ongoing support to help people feel informed and in control of their lives.

Qualifications

• Experience in customer-facing, support or advisory roles.
• Ability to listen, engage and understand individual needs.
• Comfortable working independently and with a team.
• Willingness to travel across Bristol.
• Good time management and diary skills.

Responsibilities

• Visit people in their homes across Bristol and meet NHS colleagues.
• Provide information, advice and ongoing support for people affected by dementia.
• Coordinate appointments and balance priorities in a busy service.
• Develop joined-up support with GPs and local professionals.
