Job summary

Cornwall Partnership NHS Foundation Trust in Penzance is seeking a Band 6 Dementia Liaison Nurse for an 11 month fixed term to work within the Dementia Liaison Nurse service, delivering assessment and care planning for people living with dementia in the community and care homes.

You will work with a multidisciplinary team, require NMC registration, postgraduate dementia training and post qualification experience, and contribute to reducing hospital admissions through holistic, bio-psycho-social

Qualifications

• NMC registration is required.
• Postgraduate dementia training is required.
• Substantial experience with dementia care post-qualification.
• Teaching/training experience is desirable.

Responsibilities

• Undertake with support specialist bio-psycho-social assessments for patients with dementia.
• Build strong working relationships with staff in care homes, GP practices and community teams.
• Help reduce hospital admissions by coordinating care in the community and in care homes.
• Apply knowledge of Mental Capacity Act and Mental Health Act in care planning.
