Job summary

Cornwall Partnership NHS Foundation Trust in Cornwall is seeking a Part Time Primary Care Dementia Practitioner (Band 5) to join a specialist team across Cornwall. The role supports people with dementia, their families and carers, working with the Memory Assessment Service and Dementia Older People's Mental Health to provide seamless care in the community.

You will listen, assess, and problem-solve to place solutions, preventing crises and unnecessary hospital admissions.

Qualifications

• Current professional registration with NMC or HCPC.
• Experience with dementia and mental health.
• Ability to work with families and carers.

Responsibilities

• Assess needs of people with dementia and family carers.
• Provide specialist information and advice to carers.
• Support people from diagnosis through their dementia journey.
• Help carers develop skills for caregiving.
• Promote living well with dementia.
• Coordinate care with Memory Assessment Service and other teams.
• Engage with GP surgeries to identify dementia on registers.
