Cornwall Partnership NHS Foundation Trust is seeking a Primary Care Dementia Practitioner to join a specialist team across Cornwall, supporting people with dementia and their carers within the community. You will listen, assess and put solutions in place to prevent crisis and hospital admissions, working with GP surgeries and the Memory Assessment Service.

Applicants should have transferable skills from Mental Health, Dementia or Physical Health settings, with backgrounds in nursing,

Qualifications

• Experience in dementia care or related mental health fields.
• Ability to assess and provide information to carers and families.
• Strong communication and collaborative teamwork in a multi-disciplinary setting.
