Leaders in Care is recruiting an experienced Social Worker to join the Children with Disabilities Team in South Wales on a long-term contract. You will work within a supportive service that prioritises relationship-based practice, high-quality assessments, and positive outcomes for children with disabilities and their families.

Qualifikationen

• Qualified Social Worker with registration in England/Wales.
• Experience with Children with Disabilities or safeguarding.
• Knowledge of relevant legislation for disabled children.

Aufgaben

• Manage caseload of children with disabilities and complex needs.
• Undertake Child and Family Assessments and statutory assessments.
• Complete Care Act and Short Break assessments where appropriate.
• Develop, implement and review child-centred care and support plans.
• Collaborate with families, schools, health professionals and partner agencies.
• Assess and manage safeguarding concerns per statutory guidance.
• Support children in need, child protection and looked after processes.
• Prepare high-quality reports, assessments and court documents when necessary.
• Ensure the voices of children and young people are central to planning and decisions.
