Oxford Health NHS Foundation Trust in High Wycombe is seeking qualified Nurses, Social Workers or Occupational Therapists for a Crisis Response and Home Treatment role. You will collaborate with psychiatrists, psychologists and a multi-disciplinary team to deliver intensive home-based care, while maintaining safety and patient-centred planning.

The role involves flexible shifts, risk assessment, and delivering evidence-based practice in a fast-paced environment within the Urgent Care Pathway

Qualifications

• Registered professional educated to degree level or equivalent.
• Experience in crisis/home treatment settings desirable.
• Ability to work independently with team support.

Responsabilités

• Deliver home treatment interventions with a multi-disciplinary team.
• Assess risk and determine least restrictive care pathways.
• Signpost service users to appropriate treatment options.
• Work across Saffron House and county locations as required.
