Job summary

Pennine Care NHS Foundation Trust is looking for a caring and adaptable Support Worker to join the Crisis Resolution Home Treatment Team (CRHTT) in delivering intensive, 24-hour support to service users in acute mental health crisis. This role offers an alternative to hospital admission and requires collaboration with a multidisciplinary team.

Qualifications

• Experience in a care or support role is desirable, but full training will be provided.
• Ability to demonstrate empathy, resilience and commitment to supporting people in acute mental health crisis.
• Be able to work effectively within a multidisciplinary team.

Responsibilities

• Assist qualified practitioners in delivering intensive, 24-hour support to service users in acute mental health crisis.
• Help service users engage with community resources and promote independent living.
• Provide practical and emotional support, safeguarding, and relapse prevention.
• Maintain accurate documentation in line with Trust policies and procedures.
• Work a rolling rota covering day and night shifts and liaise with other statutory and non-statutory agencies.
