Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust is recruiting for a Crisis Resolution and Home Treatment Practitioner (NHS AfC Band 6). The role is full-time, permanent, and requires 37.5 hours per week with a 24/7 rota.

You will work in the crisis team providing urgent assessments, triage, and safe, person-centred care planning across shifts. The post involves multidisciplinary collaboration with nurses, psychology, and other colleagues, and offers opportunities for cross-cover within

Qualifications

• Registered Mental Health Nurse, Social Worker or OT with appropriate registration.
• ENB 998 or mentor qualification or willingness to complete.
• Evidence of post-registration training.
• Safeguarding training.
• Non-medical prescribing.

Responsibilities

• Triage all mental health referrals to determine priority and clinical pathway.
• Carry out full assessments including risk assessments.
• Refer to the most appropriate team/service using available resources.
• Identify treatment-based interventions and ensure best practice.
• Act as gatekeeper to informal admissions to hospital.
