Job summary

The Crisis Resolution & Home Treatment Team in Southampton supports individuals with acute mental health needs requiring intensive input to remain in the community. The team collaborates with ward colleagues at Antelope House to reduce prolonged hospital stays by offering hospital-at-home support.

We seek a Mental Health Support Worker committed to patient-centred care, supported by a multidisciplinary team to deliver intensive, recovery-focused interventions for adults in the community.

Qualifications

• Potential candidates should ideally have relevant mental health community experience.
• Excellent communication skills and a positive attitude to care of adults within the community.

Responsibilities

• To observe, report and record changes in the service user's mental health, physical health and behaviour.
• Identify early signs of deterioration and changes to risk presentation.
• Be actively involved in assessment, planning, implementation and review of individual service users' care plans.
• Build rapport with service users whilst maintaining personal and professional boundaries.
• Support service users in engaging with the agreed care plan formulated within multi-disciplinary meetings.
• Assist the individual to access appropriate services provided on a regular and consistent basis, and to ensure the individual understands their pathway of care across sector/agency boundaries.
• Promote service users in the community by maintaining knowledge of and links with community resources.
• Provide regular and practical support to service users and their carers in developing independence and maintaining dignity and self-respect.
• Assist with planning daily activity scheduling and home management.
