East London NHS Foundation Trust is seeking a Crisis Pathway Mental Health Practitioner to join the 24/7 CRHTT in Bedford. You will assess adults in mental health crisis, create formulations, manage risk, and coordinate home treatment or crisis house care.

You will work within a multidisciplinary team, provide gatekeeping, mindful safeguarding, and facilitate safe discharge from hospital where appropriate, using psychosocial interventions to support recovery and reduce readmissions.

Aufgaben

• Assess adults in mental health crisis from multiple sources, including GPs and hospital wards, and provide home treatment where appropriate.
• Conduct comprehensive risk assessments and update risk at each contact.
• Provide expert advice on management, treatment interventions and diagnosis during mental health crisis to minimise risks and promote recovery.
• Support acute hospital wards and facilitate early discharge or leave, reducing length of stay when possible.
• Use psycho-social interventions to improve mental health outcomes and enable service user-led recovery and independence.
